LGV/PCV Learners

Useful documents for people who are planning to get an LGV or PCV licence or already have one.

If you need to book a medical you can visit your local GP or try Fit 2 Drive Medicals. Additional information on Driver Medicals can be found here.

D2 – Provisional Application Form is not a downloadable document. You can order a printed copy directly from the DVLA here or contact us and we can help.

D4 – Medical Examination Report (LGV or PCV) Licence – can be downloaded.
Some doctors will be able to fill in both the vision and medical assessment sections of the report. If your doctor is unable to fully answer all the questions on the vision assessment, you must have it filled in by an optician or optometrist.

You can also collect your D2 and D4 forms from SP Training or from your local Post Office
